Printable Medical Binder for Elderly Parent: Medication List, Doctors, Insurance, Expenses

When the papers pile up and everyone asks you, it helps to have one place to look.

Discharge sheets, medicine lists, insurance letters, receipts and sticky notes with a phone number on them end up in different drawers. Then a nurse asks when the last medicine change was, or a sibling asks what the doctor said, and you're expected to remember. This binder gives each of those things its own page. Fill in what you know, leave the rest blank, and add to it as you go. When someone else steps in to help, they can open the binder instead of calling you.

Inside pages are dark type on white with a thin header band, so they're low-ink to print. Hole-punch them for a binder, or print only the pages you need and reprint any log as it fills up.

A few notes

• Copy medicine details exactly as printed on the pharmacy label, and ask the pharmacist about anything unclear.
• The expense and mileage pages are for your own records. Ask a tax professional what, if anything, you can deduct.
• The "where papers are kept" page is only a list of locations. It is not legal advice.

Format

2 PDF files: US Letter and A4, with the same 34 pages.

Sizes

US Letter (8.5 x 11 in) and A4 (210 x 297 mm), each laid out for its own paper size. Same pages in both. Use Letter in the US and Canada, A4 almost everywhere else.

How to download and print

1. After checkout, your download is ready right away. The link is also in your email receipt.
2. A computer is easiest for saving the files.
3. Open the PDF and print at 100% or "Actual size" (not "Fit to page") on Letter or A4 paper. A home printer or a local print shop both work.

Print as many copies as you need, and share them with your family, church, class, or group. Please don't resell the files or printed copies, post them for download, or use them in products for sale.
