Caregiver Medication Log Spreadsheet, Excel and Google Sheets: Appointments, Doctor Summary

A parent's medicines, visits and daily care, in one calm place.

When a parent's care is split between text messages, sticky notes and memory, it's hard to say which medicine was given, what the doctor said, or how the nights have really been. This spreadsheet gives the whole family one place to record it. Each helper picks from dropdowns and adds their name, the Dashboard counts what was written down, and before a visit you print a one-page Doctor Visit Summary for the last 7 to 90 days. It records and counts. It doesn't score, judge or send alerts.

What's included

• Dashboard: a "This week at a glance" strip, number cards and simple graphs, week by week and month by month
• Calendar: a month view with Month and Year dropdowns and a "Coming up this week" list
• Week View: a printable week with a seven-day medicine grid (G given, R refused, H held) and a weekly care overview
• Doctor Visit Summary: one printable page covering the last 7, 14, 28, 30, 60 or 90 days
• Med Log: one row for each medicine time, with G, R or H, a note and who recorded it
• Daily Care Log: one row a day for meals, drinks, sleep, mood, personal care and notes, mostly dropdowns
• About This Person: emergency information and "About my loved one", ready to print for a new helper
• Setup: helpers, current medicines (copied as on the label), medicine changes and short lists
• Incidents, Tell the Doctor, Visit Notes, an optional Vitals Log and optional Expenses with monthly totals
• A filled-in EXAMPLE file with made-up entries, so you can see every tab working before you start

Format

• One zip file: two .xlsx files (blank and EXAMPLE) plus a PDF Start here guide
• Works in Microsoft Excel and in Google Sheets (free with a Google account). A computer is easiest for setup.
• One loved one per file, 12 months per file. Summary pages print on Letter or A4.

How to download and open

1. After checkout, your zip file is ready to download right away. The link is also in your email receipt.
2. Unzip it on a computer and open the Start here guide.
3. Excel: double-click the blank .xlsx file. Google Sheets: upload the .xlsx to Google Drive, open it, then choose File, then Save as Google Sheets.
